From a foreign diploma to the BIG register.

Five phases: medical Dutch, diploma recognition, the BI-toets, BIG registration and your first job as a doctor or dentist. You always know where you stand and what comes next.

In short

Who is it for?
Doctors and dentists with a foreign diploma who want to work in the Netherlands and need BIG registration.
What do we do?
We build the two parts you prepare yourself: medical Dutch from A0 to B2+, and BI-toets preparation with 4.137 questions, each with an explanation and a source.
What don't we do?
We are not a recruitment agency and we do not place anyone. Diploma recognition and BIG registration are decided by CIBG and CBGV; we show you each step and who decides it.
Where do I start?

A real conversation from the course, level B1 → B2+: a doctor asks consent for software that listens in and writes the report. Tap a marked word for its meaning.

And then the exam.

BigMentor has 4,137 practice questions for doctors and dentists. After every answer: why it is right, and the guideline, law or textbook it rests on. Try one.

Today's practice test, 8 questions → No account needed. The BI-toets is in Dutch, and so are the questions.

Our Story: From Gratitude to Action

A moment
I will never forget.

After months of anxiety, I stood in a park in Amsterdam and watched my children play peacefully on the playground. There were no sirens, only the calm rhythm of the city. In that moment, I felt not only relief but deep gratitude, and I knew the best way to repay this country was to apply our medical expertise where it is needed most.

Everything you need to know about the BIG registration and our platform.

BIG stands for Professions in Individual Healthcare (Beroepen in de Individuele Gezondheidszorg). In the Netherlands, BIG registration is legally required for all healthcare professionals who want to use protected titles or perform reserved procedures. Without a BIG registration, you are not allowed to practice as a dentist, doctor, or nurse in the Netherlands.
It varies a lot from person to person. Two things set the pace: the B2+ language certificate, which you already need for the application, and the waiting time for the BI-toets. According to the UAF, that wait has risen to about two years for doctors; for dentists there is currently no waiting list. A complete dossier at the first submission saves months.
Among other things: a legalised diploma with apostille, a certified transcript, a Certificate of Current Professional Status from your home country, a valid ID, and language certificates: Dutch at B2+ and English at B2 (reading). Documents in another language need a sworn translation. The current list is on bigregister.nl.
The language requirements vary by profession. Level B1 is required for mbo nurses and VIG (plus mandatory English reading). Level B2 is required for hbo nurses (plus mandatory English reading). Level B2+ is required for doctors and dentists, along with English B2 (State Exam NT2 Programme II is B2 and not enough for them; only a B2+ certificate from the list the CIBG accepts counts, see bigregister.nl). Certificates must not be older than 2 years and a course attendance certificate is insufficient.
Yes. Temporary protection (Directive 2001/55/EC) allows you to work in the Netherlands, but to work as a doctor or dentist you go through the same BIG procedure as everyone else. Our medical Dutch course and BI-toets preparation are open to you; you submit the application itself to the CIBG.
The CBGV assessment is free. BIG registration costs €85 (€121 from 1 January 2027). The BI-toets currently costs €1,500 for dentists and €1,700 for doctors; under the tariff proposal that went through public consultation, this rises on 1 March 2027 to €2,733.25 and €5,371.50. You also pay for legalisation, sworn translations and the language certificates yourself.
With two parts of the route that we build ourselves: the medical Dutch course from A0 to B2+ (taal.mentoramedic.eu) and BI-toets preparation (bigmentor.nl), with 4,137 exam questions, each with an explanation and the source it rests on, and practice consultations with a virtual patient. The steps at the CIBG, the CBGV and your employer are not ours; we lay them out clearly. We do not act as a recruitment intermediary.
Yes, under Article 38 of the Dutch BIG Act (task delegation and supervision), it is possible to work clinically in supporting or delegated roles, such as medical assistant or basic doctor under supervision. A BIG-registered colleague maintains overall responsibility for the procedures and must verify your competence.
The supervision route is a temporary working arrangement where you perform delegated clinical tasks under the direct supervision of a BIG-registered colleague, without independent license to practice. Full BIG registration is the final goal where your foreign diploma is officially recognized, and you can practice independently under your protected title after meeting the language requirements and passing any required BI-toets.
This programme is for doctors with a diploma from outside the EU/EEA. If your diploma is from the EU or EEA, visit mentoramedic.eu for the track that fits your situation.
